• Income Information for the past 12 months (tax forms, W-2 forms, paycheck records, pay check stubs, etc.)

• Social Security Card or numbers (if applicable)

• Medical Assistance Numbers (if applicable)

• TANF/SSI Documents (if applicable)

• Dental Information:  Documents from most recent (in the past 6 months) health exam or attached documents

• Medical Information:  Documents from most recent (in the past 6 months) health exam or attached documents

• IEP/IFSP/IIIP:  Document for children with special needs (if applicable)
